The Feedback and Outcomes Follow-Up is composed of 4 parts:


  • Feedback Questions - a set of 8 questions that aim to assess the client's overall opinion of the experience, the facilitator, and the center. This includes 6 questions that were adapted from a recent study out of OHSU to measure the impact and safety of the session and 2 questions that measure the client experience.


  • The World Health Organization's Well-Being Index (WHO-5) - a five question measure of current mental well-being. These post-journey results are compared to the baseline results measured before the journey.


  • Patient Health Questionnaire (PHQ-2 & PHQ-9) - the clinical standard for measuring depression. The PHQ-2 includes the first two items of the PHQ-9 and acts as a first-step approach to screen for depression. If a client screened negative on the PHQ-2 baseline measured before the journey, they will skip this portion of the questionnaire. Otherwise, they will take the full PHQ-9 and this result will measure the severity of depression and will be compared to baselines to measure changes.


  • Generalized Anxiety Disorder (GAD-2 & GAD-7) - the clinical standard for measuring generalized anxiety. The GAD-2 includes the first two items of the GAD-7 and acts as a first-step approach, similar to the PHQ. If a client screened negative on the GAD-2 baseline measured before the journey, they will skip this portion of the questionnaire. Otherwise, they will take the full GAD-7 and this result measures the severity of anxiety and will be compared to baselines to measure changes.



We recommend clients complete this at least 2 weeks after the journey, as many of the questions ask the client how they felt over the last 2 weeks. The Feedback and Outcomes Follow-Up can be found in the Integration section of a Session. If marked "Required", it is automatically sent to clients 2 weeks after the Session date.
